To answer my own question, the appdata pb files seems to be built from the files in the game directory pb folder. I deleted the appdata folder and then extracted the pbbans files into the game directory. After running the game the appdata pb folder had been rebuilt based on the new files in the game directory.

What about the PB files in the AppData\Local\PunkBuster\COD4 directory? Ignore them?
