adding server

I have tried serveral times to add our new server to my list on managed server but it will not add the server it gives me this responce "Server 69.12.91.37:28960 is already streaming"

any ideas on how to add this server ?

The server is currently in another clans account:

I'll remove the server from that account, as it appears to be not in use anymore.
