Maybe same situation as with ET one week ago.
Try to find in logs:
Kicking for new violations has been temporarily disabled per PB Master Servers - use the pb_sv_enablekicks command to manually re-enable kicks.

If you have any problem with updating your PB server in CoD2 , please do update manually.
Here is written how to.
When you finish updating , execute over rcon PB_SV_Restart.

our check for rate is 2500-25000

Its not from pbbans. Even if you set rate over 25000 it has no effect. Its not important check but it may help you to get a rid of laggers

Open your ET config and look for seta rate 40000 or rate 40000 and change it 25000. Another way is to remove this cvar check.

Either broken drivers or ss blocker. glReadPixels is function to read content of framebuffer.
Weird , I thought that cod2 is using direct3d.

I've never played CoD2 so this is only my personal opinion.
I would use pb_sv_cvar com_maxfps IN 40 125.
