Yes, it happens, can be a bug as well of pb-hack origin. Get the guid from the pb server logs, 100% secure way is to calculate the MD5 of the pbss [example: http://www.pc-tools.net/win32/md5sums/ Note: this is NOT pb_sv_md5tool!] and search the logs for the MD5 sum.

I never used any games' functions to ban, it's unreliable and more or less poor supported, always Punkbuster-Server.
