Looks to me like someone has FTP access to your console_mp_server.log file, the file can be called by anyone through internet or someone's good at guessing rcon passwords.

CoD4 most likely, or CoDWW Anyway, PunkBuster was unable to send a ban command to the server to ban someone from it. Try reinstalling punkbuster.

It's a hack. Any violations above #50000 are known hacks. To be more specific: #50000 - #59999 are aimbots.

The players Diceeee and Tigger are already on the MBi of PBBans. They got a ban on your server as their GUIDs have been seen with hacks before, either on your server or another PBB streaming server.
